Controlled Drugs Register

This Controlled Drugs Register logbook is setup in a grid table format for efficient and organized entries. Providing 27 pages of tables, this logbook is separated into two sections: Section 1 (11 pages) - Drugs obtained from authorized supplier or confiscated from seafarers. These tables provide 13 entry spaces each and each table contains: date, suppliers name/address, amount obtained and name of drug/form in which obtained. Section 2 (16 pages) - Drug dispensed or disposed of. These tables provide 14 entry spaces and each table contains: date, suppliers name/address of person given drig, right of person to have drug, amount supplied and drug and form in which supplied.
